Mohgwyn Palace is an underground region hidden beneath the Consecrated Snowfield, accessible only through specific means. It is the domain of Mohg, Lord of Blood, and serves as the endgame destination for players following the Lord of Blood's dynasty storyline. The palace is bathed in crimson light, with blood waterfalls and pools everywhere. Despite being optional, it offers some of the best bleed-focused gear in the game, including the White Mask, Lord of Blood's Exultation, and Mohgwyn's Sacred Spear. The Palace Approach Ledge-Road is also the most popular rune farming spot in Elden Ring, making this region invaluable for leveling.
The most famous rune farming spot in Elden Ring. From this Grace, use any bow to shoot the bird on the cliff, causing it to run off and die for ~11k runes per kill.

Mohgwyn Palace is technically part of the underground river network, connected to the Siofra River region.
The main boss of the palace. A powerful bleed-focused boss with a unique phase transition (Nihil) that heals him while draining your HP. Drops Mohg's Great Rune and the Remembrance of the Blood Lord.

Dropped by Mohg. A great spear with a powerful weapon skill that covers the area in bloodflame.
Dropped by Eleonora at the Church of Vows (part of Yura questline). When added to the Flask of Wondrous Physick, negates Mohg's Nihil phase transition damage.
Found in the palace. Used to upgrade special weapons to +10.
The fastest way to reach Mohgwyn Palace is via Varre's questline: defeat the Great Rune bosses, use the Festering Bloody Finger three times (invade NPC worlds at Writheblood Ruins), talk to Varre, and receive the Pureblood Knight's Medal. Use the medal to teleport directly to the palace. Alternatively, use the teleporter in the Consecrated Snowfield at the far northwestern edge of the map. Once inside, explore the Approach Ledge-Road (rune farm spot) before heading to the Dynasty Mausoleum for the Mohg boss fight. Fextralife's Mohgwyn Palace page has detailed farming strategies and item locations. Wikipedia's Elden Ring lore covers Mohg's connection to Miquella and the Formless Mother.
Pro Tip
For the Mohg boss fight, the Purifying Crystal Tear is essential — it completely negates the damage from Mohg's Nihil phase transition. Without it, you will need extreme healing to survive. Also, Mohg is highly vulnerable to bleed despite being the Lord of Blood, so using a bleed weapon against him is ironically very effective.
